If your needs can't be met at home, the council's social care department and the hospital will work with you, your family and carers to look at your options.

Your assessment will recommend the type of care home that will best meet your needs. You'll then be given a list of suitable homes to choose from. If the home you choose doesn't have a room available, you may need to move temporarily to another home while waiting for a suitable one.

You can't wait in hospital for a vacancy in your home of choice. Hospitals are places to go for short-term episodes of treatment that cannot be provided in the community. Spending unnecessary time in hospital can be detrimental to your health, and can lead to a loss of confidence and independence.
