Social care support from home

Overview

Social care support from home is social care support provided in your own home or in the community to help support your independence. It’s sometimes also called:

  • care at home
  • home care
  • home help
  • home support

It may involve regular visits from a support worker and social care support, health and housing services like:

  • personal and nursing care
  • general cleaning, heavy housework, small repairs and gardening
  • shopping
  • meal preparation
  • meals on wheels or frozen meals delivery
  • collecting prescriptions
  • assistance with taking prescribed medication
  • paying bills and assisting with finance management
  • laundry, like washing in your home or a laundrette
  • equipment and adaptions, like telecare and home improvements
  • reablement – this usually lasts up to 6 weeks and means you’ll get personal care to help you live independently at home after leaving hospital

Ask your local pharmacy if they could deliver your prescriptions, if you find it difficult to pick up your medication.
