Assessment of your care needs

Who’s involved in an assessment of your care needs

The most important person involved in the assessment is you.

The person who carries out the assessment with you may be a:

  • social worker
  • district nurse
  • occupational therapist
  • another social care professional

They may be the main social care professional involved in your social care support. But they might also work with other social care professionals to prepare your care plan.

They must ask for your written permission to share specific information with other social care professionals. Information is shared to make the best assessment possible. This is sometimes called a shared assessment.

Who can come with you to the assessment

You can choose to have a friend, family member or advocate with you for the assessment. Find out more about advocacy.

If you get support from a relative or friend, they can also attend your assessment as your carer. It’s important that your carer is clear about the level of support they can offer you. Some of the carer’s needs might also be in your care plan, for example short breaks for your carer. 

Your carer can also have a separate personalised plan to identify any support they need while caring for you.
